For my tuppence I'd always try and find out if anyone was working in the relay room at the time, in-case it was a 'fat fingers' mistake. The building entry log is helpful, assuming it's being filled in appropriately.
Similarly was there any project or construction work going on nearby. Sometimes the people working on the project can be less familiar with the consequences of their accidents.
