It seems the UK is often slow and very resistant to implementing new technologies or changing applications to suit needs. Red For Danger gives many examples of how Safety and good sense can be clouded for many reasons.
However, there is also a theory that the safer you make a system, the more probable the effects of an incident/accident increase. If you use a car as a simple example, the inclusion of ABS etc., means the fuzzy pink [#] SIL0 systems come to rely on them to save the day - not a safe assumption - hence, speed goes up, failure occurs, outcome is significantly more.
Does that mean we should remove the safety systems? Of course not. We may ask the question that is the percieved value for these systems actually met? Should we avoid interim measures and spend the time, effort and money in developing the right solution?
